I will continue to maintain my phone number for a period of time but I do not check that number daily. If you are needing records or for me to consult with your new therapist, fill out a Release of Information [# 6 under Forms tab] and email it or fax it to me. The email address and fax number are above AND on the form itself. If you are wanting a referral for another therapist or have any other questions, please email me and I will get back to you.
